Your Path to Care

We've designed a straightforward process to help you access the mental health care you need with minimal stress and maximum support.

Initial Contact & Screening

Complete our Pre-Intake Form to ensure we are the right fit for your unique case. Our team will then reach out to discuss your needs and schedule your first appointment.

What to Expect:

  • Insurance verification and payment options
  • Scheduling your first appointment

Complete Intake Forms

Before your first appointment, you'll complete secure, HIPAA-compliant intake forms that help us understand your history, current concerns, and treatment goals.

Forms Include:

  • Medical and psychiatric history questionnaire
  • Current symptoms and concerns assessment
  • Medication history and allergies
  • Insurance information and consent forms
  • Treatment goals and preferences

Note: Please complete forms at least 24 hours before your appointment to ensure we have adequate time to review your information.

Your First Appointment

Your initial consultation is typically 60 minutes and focuses on getting to know you, understanding your concerns, and beginning to develop a personalized treatment plan.

During Your Visit:

  • Comprehensive review of your history and current concerns
  • Discussion of your goals and treatment preferences
  • Mental health assessment and evaluation
  • Initial treatment recommendations and options
  • Opportunity to ask questions and address concerns

Ongoing Care & Support

After your initial appointment, we'll work together to implement your treatment plan with regular follow-ups, medication management if needed, and ongoing support.

Ongoing Support Includes:

  • Regular follow-up appointments (frequency based on your needs)
  • Medication monitoring and adjustments as needed
  • Access to patient portal for secure communication
  • Crisis support and emergency resources
  • Coordination with other healthcare providers as needed

Important Information

Appointment Times

Initial appointments are 60 minutes. Follow-up appointments are typically 30-45 minutes. We always allow adequate time to address your concerns thoroughly.
